Purpose: Use this e-form to request a Change of Education Level I-20 if you have been admitted to a new degree program at UArizona.

Purpose: Use this e-form to extend the end date of your I-20 or DS-2019 if you need more time to complete your degree.

Personal Information Update

Curricular Practical Training
Purpose: Use this e-form to apply for permission to engage in curricular practical training (CPT).

F-1 Social Security Letter
Purpose: Use this e-form to request a letter that you can present to the Social Security Administration (SSA) to apply for a Social Security Number (SSN).

On-Campus Employment Authorization
Purpose: Use this e-form if you are a J-1 student with an on-campus employment offer.

Optional Practical Training
Purpose: Use this e-form to apply for an I-20 recommending optional practical training (OPT).

OPT Reporting

Concurrent Enrollment

Full-Time Enrollment Exemption
Purpose: Use this e-form to request permission to enroll less than full-time during your last semester at the University of Arizona.

Requires academic department approval. Students with more than one major must submit the Supplemental Advisor Information e-form.
Graduate Coursework Completion
Purpose: Use this e-form if you have completed all coursework, or coursework and required 900-level units (thesis/dissertation/recital) and would like to be considered full-time with fewer than 9 units.

Medical Reduced Course Load
Purpose: Use this e-form to request permission to enroll less than full-time due to a medical condition.
Average Processing Time: 10 business days
Required Documents:
- A letter (printed on letterhead) from a licensed medical doctor, psychiatrist, doctor of osteopathy, licensed psychologist or clinical psychologist recommending a partial or full drop due to a medical condition (specific details about the condition are not required).
Individual Taxpayer Identification Number
Purpose: Use this e-form if you are receiving a taxable cash scholarship and need to apply for an Individual Taxpayer Identification Number (ITIN) for federal tax reporting.

Authorized Early Withdrawal
Purpose: Use this e-form to request record termination for Authorized Early Withdrawal.

Transfer Out

Purpose: Use this e-form to request an e-Letter of Expense.

e-Letter of Invitation
Purpose: Use this e-form to request an e-Letter of Invitation for your friends and family.
Average Processing Time: 5 business days
Special Note: The U.S. Department of State has indicated that letters of invitation are not required for visitor visas: “Visa applicants must qualify on the basis of the applicant's residence and ties abroad, rather than assurances from U.S. family and friends. A letter of invitation or Affidavit of Support is not needed to apply for a nonimmigrant tourist visa. If you do choose to bring a letter of invitation or Affidavit of Support to your interview, please remember that it is not one of the factors that we use in determining whether to issue or deny a nonimmigrant tourist visa.”

e-Verification of Immigration Status
Purpose: Use this e-form to request an e-Verification of Immigration Status.

If you were last enrolled between Fall 2004 and Fall 2013, we may have archived copies of your documents that are not accessible under the Documents tab. To request copies of archived documents, submit a Record Request in MyGlobal.
We cannot guarantee we have copies of all the documents you are seeking. Ultimately it is your responsibility to retain copies of your documents.
